This form is called from the Field Attributes form, for Listbox or Radio Group field types, when you press the edit button 
next to the List Choices entry field.

Press the Add button and add as many entries as you wish. These will be displayed in the associated listbox or radio button group.
Use the arrow buttons to alter the order of the items in the list.
